Hertfordshire Community NHS are currently looking to recruit into our newly formed HomeFirst teams within East and North Hertfordshire and West Hertfordshire. 

HomeFirst is an innovative new partnership model of community care that supports patients’ holistic needs in their home. This fully integrated service combines community health, social care, mental health and voluntary sector agencies within Hertfordshire which provide the right treatment at the right time in the right place.

The patient is central to this process so access is easy, the service is responsive to the patients’ needs, self-care is promoted, adherence inspired, resulting in a high quality service in which health and social care outcomes are maximised.

You will be joining a forward thinking team who are committed to providing high value healthcare in patients’ homes and the within the community, delivered by skilled and empowered staff. This is an exciting opportunity for experienced professionals to join the HomeFirst teams and drive integration forward.

Posts will be available within Upper Lea Valley

A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum payment of £961 and a maximum payment of £1,665 per annum
